Assessment summary
Introduction
Mrs Chungsoon, aged 77, was referred to the Aged Care Assessment Services (ACAS) for access to eligible aged care services and planning for the future. An assessment occurred face-to-face on 04/07/25 in Chungsoon’s home in the presence of her partner Sejin.
Situation
Chungsoon was diagnosed by a geriatrician with early stages of Alzheimer’s disease and there has been a gradual decline in her cognition which has impacted on her ability to carry out complicated tasks independently.
As reported by Sejin and Chungsoon, her medical history includes, but is not limited to, high cholesterol, cognitive impairment (Alzhimer's Disease) and sleep disorder associated with Alzheimer’s disease (unsure of the specific disorder).
Background
Chungsoon is of Korean background and lives her partner Sejin in their own home. She has regular contact with her son that lives nearby, but he is currently working full time and has family commitments. Sejin is Chungsoon’s main carer and assists with cooking, transport, medication supervision and ad hoc care needs.
There are no formal services in place.
Assessment
Behaviour/cognition: Alert and oriented. Was engaged in conversation. Sejin reported that Chungsoon has difficulty with her short-term memory.
Social/emotional: Chungsoon goes to the church over the weekends. Occasionally would go on social outing with her friends and has dinner with her son on a weekly basis.
Personal care: Independent with shower chair in place.
Toileting: Independent.
Medications: Chungsoon organises her medication in a dosette box. Requires supervision from Sejin and occasional reminding to take her medications.
Mobility/transfers: Independent unaided and independent with transfers. When pain is present in her feet and knee it is more of an effort for Chungsoon to mobilise. Has had multiple falls/near falls within the past 12 months due to legs giving way.
Finance/legal: Sejin manages. No Enduring Power of Attorney and Advanced Care Directive in place but Sejin will look into it for Chungsoon.
Transport: Drives locally, only in familiar areas and during the day. Sejin would assist at other times or her church friends.
Shopping: Independent.
Cleaning: Shares cleaning tasks with Sejin but reported it was an effort at times.
Meals: Relies mostly on Sejin as cooking has become more complicated for Chungsoon. Occasionally she would attend to light meal preparation.

Validated assessment tools used: Dukes Social Support Index; de Morton Mobility Index (DEMMI) – Modified; Step 1 and Step 2 General Practitioner (GP) assessment of cognition.
Recommendations:
Chungsoon wants to remain at home for as long as possible. Following assessment, Chungsoon would benefit from approvals for a Home Care Package, residential respite care and permanent residential care.
Chungsoon reported that she is currently managing with current care arrangements. Consent to put Home Care Package (HCP) on not seeking services. Please call My Aged Care to put HCP on seeking services when current care arrangements are no longer sustainable.

A copy of Chungsoon’s support plan has been forwarded to Sejin via email with consent.
Sejin is encouraged to register with the Carer Gateway, 1800 422 737 for ongoing information and support in her/his carer role.
