I am writing about your assessment by Kimberley Lim of the Department for Health and Wellbeing ACA Metro East & West SA Aged Care Organisation on 04 July 2025.

After careful consideration you are approved as eligible to receive the care types listed below from 04 July 2025.

Unless indicated below, these are permanent approvals which allow you to receive Australian Government subsidised aged care. 

Residential Care

  Permanent Residential Care.

  Residential Respite Care. This approval allows you up to 63 days of subsidised residential respite care in a financial year.  If you need respite care for longer than this, you (or if you are receiving residential respite care, your provider on your behalf) can apply to an ACAT for an extension (up to 21 days). More than one extension can be granted. 

Home Care

  Home Care Package Level 2.

  Your priority for home care services is Medium. At assessment you indicated that you are not seeking home care services at present. Therefore, you will not be placed on the national priority system for access to home care.

You can call My Aged Care on 1800 200 422 to re-enter the national priority system for access to home care services at any time in the future. You will not be disadvantaged if you choose this option as you will re-enter and then be assigned a package based upon your existing home care approval date and priority for service. The time you have waited will be counted from your initial approval date.

The reason for these decisions, other approval conditions and your rights to have these decisions reviewed are explained in the next pages of this letter.

REASONS FOR THE DECISION

The decision to approve you for Home Care Package Level 2 and Residential Permanent and Residential Respite Care has been made under section 22-1 of the Aged Care Act 1997.

You were assessed as having care needs that require the provision of care and meet the other criteria for the care approved.

The decision to prioritise your needs for home care as Medium has been made under section 22-2A of the Aged Care Act 1997

 

EVIDENCE

In reaching my decision I have considered your medical, physical, psychological and social circumstances as indicated by the following evidence:

-  Evidence of your medical condition provided by Mrs Chungsoon Pak and Mr Sejin Pak.

-  Assessment information indicates you need help to perform daily living tasks.

 

Your priority for home care services at the Medium category has been determined using the information obtained during your assessment. 

 

APPROVAL CONDITIONS

In addition to the conditions set out in the covering letter:

❖      In a small number of cases, approval may be revoked if reassessment shows that a person is no longer eligible for the type of care they were approved for. If your approval is likely to be revoked, we will tell you in writing and you will get the chance to tell the Department of Health why you think your approval should not be revoked.

7월달에 assessment가 있었는데 이제야 그 결과들 듣게 되었네요. 그런데 7월 당시와 10월 이후와는 정순님의 상태가 너무 많이 바뀌었어요. 7월에는 레비소체 치매라는 것의 의미를 몰랐어요. 기록에는 알츠하이머와 기억력 문제를 말하고 있어요.

    7월 당시 “현재는 서비스 신청 안 함”으로 되어 있어, 그대로 두면 실서비스가 붙는 속도가 늦어집니다. “원할 때 다시 신청하면 초기 승인일(7/4/2025)부터 기다린 시간은 인정”된다고 승인서에 적혀 있습니다. 지금이 바로 그 “원할 때”일 가능성이 큽니다.
