About cardiac rehab
Cardiac rehabilitation is suitable for people who have:experienced
a cardiac arrest, heart attack, angina
had a cardiac stent, bypass or valve surgery.
Some programs also cater for people living with heart failure, those who have had a pacemaker or implantable cardioverter defibrillator (ICD), and who are living with a heart rhythm problem such as atrial fibrillation.

Our cardiac services directory includes a list of cardiac rehabilitation programs operating across Australia, with descriptions of how the program is run, when they run, any costs and referral/contact details.
Please note some programs included are exercise only – so that they are best to attend after you have completed a Phase 2 outpatient program.

Change the search radius, from 5km up to 50km, to see services within your chosen distance.
Once the results appear, click ‘View service’ to see more details about each program.
Statewide telehealth cardiac rehabilitation programs
Some states and private health insurers operate telehealth-based cardiac rehabilitation programs. These programs use phone calls and video conferencing to deliver their service. This means that you can access from your own home. For a list of these programs please see here:
Improving the quality of cardiac rehabilitation
The Heart Foundation is working closely with SOLVE CHD and ACRA to improve the availability and quality of cardiac rehabilitation programs across Australia.
On the directory you will see some programs have ACRA/ICCPR Provider Certification status. These providers have completed additional training through the ICCPR International Council of Cardiac Prevention and Rehabilitation, supported by ACRA.

Solving the long-standing evidence-practice gap associated with cardiac rehabilitation and secondary prevention of coronary heart disease
SOLVE-CHD is a NHMRC Synergy Grant, a multidisciplinary research team of researchers and clinicians with allied health, nursing, cardiology, public health, health economics and psychology backgrounds. Our goal is to transform post-discharge secondary prevention and reduce the burden of heart disease by decreasing deaths, hospitalisations and costs via a program of work that integrates data, technology, partnerships and capacity building.

The Australian Cardiovascular Health and Rehabilitation Association (ACRA) is the peak body which provides support and advocacy for multidisciplinary health professionals to deliver evidence-based best practice across the continuum of cardiovascular care.
ACRA’s aim is to empower health professionals to achieve optimal and equitable outcomes for all affected by cardiovascular disease.
