You can make an enduring power of attorney using the Enduring Power of Attorney Kit, developed by the Legal Services Commission. It contains all the forms you'll need and explains how to complete them.

Other way to apply

The hard-copy kit (with paper forms) is available for purchase from a Service SA from any Service SA centre (including metro and regional). They can also be purchased from service agents and rural service agents including: Clare, Millicent, Yorketown, Keith, Peterborough, Port MacDonnell, Roxy Downs, Wudinna and Port Broughton.They cannot be purchased from commissioned agents.


You can download an example copy of the kit which contains instructions and examples of what the completed forms should look like.

Registering power of attorney

You don't have to register your power of attorney. However, before your donee/s can deal with your real estate, the power of attorney must be lodged at Land Services SA.
Revoke a power of attorney

To revoke a power of attorney, you must:inform your donee in writing that you're ending their appointment
inform your bank and any other relevant groups or businesses your donee may have been dealing with that the appointment has been revoked.

If you don't do this, your donee can legally continue to make decisions on your behalf.

If your power of attorney has been lodged with Land Services SA, you must lodge a revocation with them. Instructions on how to do this are included in the Do-it-yourself Enduring Power of Attorney Kit.
