Home Support Program
Campbelltown Home Support Program (Campbelltown HSP) is funded by the Australian Government Department of Health and provides a range of low level services for older people. Using a wellness approach, we work with eligible clients to maximise their independence in the community, enhancing their wellbeing and quality of life.
Client contribution is applicable for all services as per the Campbelltown HSP Client Contribution Procedure. Fees for the 2024/2025 year are:
- Transport within the Council area: $4 one way
- Transport outside the Council area: $6 one way
- Social programs: costs vary
All Campbelltown HSP are subject to eligibility, screening assessments, availability and ongoing Commonwealth funding.
Eligibility
In order to receive Campbelltown HSP services you must:
- be a resident of the City of Campbelltown
- be a frail older person (65 years or over) or an Aboriginal and Torres Strait Islander (ATSI) person (50 years or over)
- have difficulty performing daily activities without assistance
- be assessed by My Aged Care and have a referral code.
Accessing programs and services
My Aged Care is the entry point for aged care services. They will determine if you are eligible for subsidised support, record your details, arrange an assessment and make a referral to a service provider for your services.
If you are a new client or require a new service type you will need to register with My Aged Care on 1800 200 422 and choose the City of Campbelltown as your preferred provider.
If you require interpreting services, you can access qualified accredited interpreters with the Australian Government Translating and Interpreting service, phone 131 450.
Transport service
The Campbelltown HSP is fortunate to have a dedicated group of volunteers who generously drive local residents to various appointments and activities.
Clients using the service must be able to transfer themselves to and from a car. Unfortunately we are unable to transport residents in wheelchairs. Walking aids are accepted provided they can be folded and fit safely in the car. Please contact the office to discuss your transport needs.
Occasionally, Campbelltown HSP staff may offer you a half price taxi voucher if your transport request is outside of normal operating times, or if you are returning from an appointment in the city. It is difficult for volunteers to provide a return trip in these circumstances due to parking difficulties.
Client contribution is applicable for transport services.
For more information contact the Volunteer Transport Coordinator on 08 8366 9283.
Assisted shopping service
The Campbelltown HSP provides local transport and assistance for grocery and personal shopping every fortnight. This volunteer based service is tailored to support clients with limited mobility who may have difficulty managing their shopping or lack family support to help with these tasks.
Eligible clients can access accompanied activities such as shopping, bill paying, banking and more.
Clients must be able to get in and out of the vehicle without any assistance.
